Transitions - Moving from one program (or realm of service) to another
- An Overview of the Process
- 2 years: The process may begin.
- 2 years, 3 months: Early Intervention Colorado will contact school district and notify Child Find of the child’s approaching third birthday.
- 2 years, 6 months: With parent’s permission, Early Intervention Colorado refers child to Child Find if he/she has unmet goals on his/her IFSP.
- At least 3 months prior to 3rd birthday:
- Service coordinator will follow-up regarding the transition plan that you have outlined on your child’s IFSP.
- Child Find evaluation
- Service coordinator should ensure that all the people you have requested will be present at the meeting.
- With parent’s consent, service coordinator will send child’s IFSP to the Child Find team.
- By age 3: You and your local school district will develop an initial service plan (IEP).
- No later than 3rd birthday: Child will transition out of Early Intervention services
- Available Options at Age Three
- School district preschool programs (may include Special Education or Colorado Preschool Program)
- Community preschools (e.g., CLC)
- Head Start
- Library or recreation groups/programs
- Other preschools or child care centers
- “During the year before your child turns three, it will be helpful for you to:
- Think of three or four goals you might have for your child in the next year.
- Discuss your questions, priorities and things that you want for your child’s transition with your service coordinator.
- Consider ways you would like to update your IFSP.
- Consider visiting the various service options in your community as a part of making your decisions.
- If you are considering preschool special education services, take time to learn about this program.”
